I printed out on computer - that is all I did. I found a graphic I liked and printed it out. I had to size it on my printing program to make it fit but it is easy enough to do. I bought the debit card holder at a yard sale.

Possible Source - Debit Card Holder
Not sure if it is the same thing that you used but it was the right size. At my local Kroger's grocery store at the pharmacy the offer FREE child and adult health history records. Each in it's own debit card size plastic holder. Might be a place to check out if you are interested in making these.

Gosh darn it I don't have a Kroger here wouldn't you know?
